Is anyone getting this error when sending forms to clients via kiosk?

KNOWN ISSUE: Client Dashboard SQL Error Message
KNOWN ISSUE: If you have Tickets displaying in the Client Dashboard, an error message will pop up when accessing the Dashboard. You can simply close the error and use the Dashboard as normal. Do agency admins have the ability to modify the standard Dropdown list on Clients Module/Misc Tab for Referral Source
Is there a way for agency administrators to modify the standard dropdown list for the How client found this firm? Location in the Clients Module/Misc Tab. This would help us out quite a bit for reporting purposes to have the same spelling of the referral source versus staff having to type it in. We end up with many similar referral sources but different spellings.
Client "Face Sheet"
Is anyone aware of a printable option in Procentive for a client "face sheet"? Basically a one sheet report with client's address, phone number, DOB, insurance policy info, primary diagnosis, etc. I have had a couple of requests from insurances companies for this when a fax is required for authorization.
Slow Loading Times - Again
Is anyone else experiencing lagging today? Searching for clients and changing workflow projects are two things that are really moving slowly. I have gotten reports from our providers in different areas, too - so I know it isn't an internet issue.

Medicare retirement date/benefit begin date
Does anyone know how to enter the Benefit Begin date for Medicare subscribers? When an eligibility check is run in Procentive, we see the Medicare number and Benefit Begin date at the bottom of each eligibility check. When adding Medicare to a client profile, there is no Benefit Begin date field to enter this in. However, there is a "Retirement Date." Is the Benefit Begin & Retirement Date the same thing?

Does anyone have any best practices that can be shared when it comes to the first of the year where benefits(not eligibility) need to be re-verified and releases in the client module need to be updated?
Currently, we end up with a tone of paperwork that ends up having to be scanned.

alternate client ID #'s
We have several programs that report to funders with alternate client ID #'s instead of the Procentive Client ID #. For example the MN-ITS Client ID#, HMIS Client ID# or the HCM ID# for the SLMH groups out there. It would be nice to have a spot to capture these alternate ID #'s in Procentive. How have other people handled this?
Taxonomy code for consolidated NPI
Happy New Year!
Is anyone out there with a consolidated NPI using Procentive's eligibility check module? That is, an NPI that has two separate taxonomies on MNITS (one for CD and one for mental health). How is this working? Since DHS approved our Rule 31 enrollment and added the taxonomy code, we've not been able to use our agency NPI and the Eligibility check in the client record. Please help!
