How do you bill an add on code with an authorization?

Example: a client has Medical Assistance and exceeds the allowed number of units of therapy in a calendar year. A KePro auth is obtained for the add on psychotherapy code, but when billed is sent on same claim with the primary code that does not require authorization. How do other providers handle this?

Has anyone started to use the code 90899 (Clinical Care Consultation)? I looked on the updated MHCP fee schedule and there is no rate. When I called Customer Service I was unable to get an answer as well. I hoping someone on here has started billing this code and could share what the rate of reimbursement was. Thanks!

When sending claims for Chemical Health Services to MN DHS for a Temp ADC (not an LADC yet), are we required to add a modifier for to show she is under Supervision? Such as and HN Modifier?

Missing SVD Rejections

All my secondary claims are rejecting for missing the SVD amount.  When I asked Procentive about it, I was told that there was an update that caused this field from auto-populating, and that we needed to manually enter the SVD on each claim.  

Is this happening to anyone else?  How can we get Procentive to change this back to the way it was?  With the number of secondary claims we file, this will add a considerable amount of time each day.
