How have you documented both the DSM 5 and ICD-10?

What fields or sections have you used or added to your documents to include both DSM 5 and ICD-10 for your patients?
This question is from a staff member. I am in hopes someone can help me.
The names from the tabular list are not he same as ICD10 or procentive. For example the tabular list is using ADHD, while the ICD10 uses Hyperkinetic Disorder and Procentive uses ADHD. Should clinicians be writing the tabular list name or the ICD name on the actual DA. If they go to procentive and click on the code it will come up as AHHD rather than ICD10's hyperkinetic.

I went in to the Diagnosis tab and entered both titles so that clinicians are not confused by that. We also are including both names in documentation so that it is there in event of an audit, so that there is no confusion in that situation either.

I looked up the code F90 in the WHO (World Health Organization) International Version of the ICD-10. You are right that it appears as Hyperkinetic disorders.

However, if you look in the ICD-10-CM Tabular Index as produced by CMS and the CDC, F90 is listed as Attention-deficit hyperactivity disorders.

The ICD-10 Codes in Procentive are the CDC and CMS ICD-10-CM (Tabular Index) Codes that you are required to bill in the United States.

You can use the International Version as a point of reference but you can only assign the ICD-10-CM codes as released by the CDC and CMS in the United States.
