claim frequency code

Getting electronic rejections for claim frequency code and not sure how to address this...ideas?

What code are you billing and which payer?

Hmmmm. Did you do a replacement or void claim and maybe not include the original claim #?

It was a replacement, but the original claim # was included.  ugh.

Did the claim originally deny or pay? If it denied and you're resubmitting, no replacement info is needed and it might cause a rejection. Just a thought
