Hey Tina, Unfortunately, no I have not. I just received back my first attempt. They, BCBS pmap, adjusted off basically the second hour billed amount and coded as N430 (procedure code is inconsistent with the units billed) and M67 (missing/incomplete/invalid other procedure codes(s)). Have you had any success? Frustrating.
